A koronáriabetegségekkel kapcsolatos eddigi kutatás nagy része a lehetséges rizikó fak to
rokkal foglalkozott, amelyek mindegyike kapcsolódik valamilyen módon az akut vagy krónikus negatív affektivitáshoz. A pozitív érzelmek szerepét ritkábban vizsgálják a ku
tatók a szívbetegségekkel kapcsolatban (gallo et al. 2004; Rozanski–kubzansky 2005).
matthews, Räikkönen, suttontyrrell és kuller (2004) egészséges nőknél (n = 209) vizsgálta az optimizmus és az érelmeszesedés progressziójának összefüggését. Az opti
mista nőknél gyakorlatilag nem nőtt az intima (az érfal belső rétege) vastagsága a két vizsgálat közötti három évben. A szerzők tehát a betegség korai, még szubklinikai szakaszában mutatták ki az optimizmus kedvező hatását.
előrehaladott koronáriabetegség esetében is kedvező hatást gyakorol az optimizmus.
Bypassműtéten átesett betegek közül a diszpozicionális optimizmussal jellemezhetők a műtét utáni hat hónapban feleakkora eséllyel kerültek újra kórházba új műtét miatt vagy a szívbetegségükkel összefüggő okból (scheier et al. 1999). A legnagyobb pszichés terhet jelentő szívbetegségek esetében, infarktus, illetve szívtranszplantáció után is gyor
sabban gyógyulnak fel a diszpozicionális optimizmust mutató betegek (Rozanski–
kubzansky 2005).
Az optimizmus mellett a vitalitás szerepét is vizsgálták. A vitalitás pozitív érzelmi állapot, több pozitív érzelem alkotja: öröm és érdeklődés, frissesség, energikusság érzése jellemzi, és mind helyreállító, mind megújító hatása van. ezek megjelenésekor
pozitív energia képződik, a személy rendelkezésére álló források megnövekednek, többek között a problémamegoldásra fordítható kapacitása is nagyobb lesz, emiatt hatékonyabbá, ru gal masabbá válik a megküzdése. másik jellemzője az érzelmek ha
tékony, rugalmas szabályozása. Ugyanakkor ezek visszafelé is hatnak: ha az érzelmek sza bályozása és a megküzdés rugalmasak, rendben vannak, akkor ez az állapot növeli az energikusságot (Rozanski–kubzansky 2005). Röviden, a vitalitás azt jelenti, hogy a személy energikusnak érzi magát, pozitív jóllétet él át és hatékonyan tudja szabá
lyozni érzelmeit (kubzansky–thurston 2007). kubzansky és Thurston (2007) az ér
zelmi vitalitás szerepét vizsgálta az iszkémiás szívbetegségek kialakulására (n = 6 025).
Az általános jóllét skála három alskálájával (vitalitás, pozitív jóllét és érzelmi ön sza
bályozás) mérték a vitalitást, átlag tizenöt évig követték a személyeket. A rizikóténye
zők kontrollálásával kapott eredmény szerint a magas vitalitású személyeknél ala
csonyabb volt a koronáriabetegség előfordulási esélye (RR = 0,81), csakúgy, mint a bár milyen okból bekövetkező halálozásé (RR = 0,76).
surtees, Wainwright, luben, khaw és Day (2006) a mastery érzése és a koherenciaérzés kardiovaszkuláris mortalitással való kapcsolatát vizsgálta az epicNorfolk tanulmány résztvevőinél (n = 20 323) mintegy hat év utánkövetés során. A mastery érzése azt jelenti, hogy a személy képes hatékonyan kezelni az életét és a környező világot (surtees et al.
2006). A koherenciaérzés pedig annak a tartós és dinamikus bizalomnak az érzése,
„hogy 1. az élet során a belső vagy külső környezetből származó ingerek strukturáltak, előre jelezhetőek és megmagyarázhatóak; 2. az ezek által az ingerek által állított köve
telmények kezelésére a források elérhetőek; és 3. ezek a követelmények kihívások, amelyekbe érdemes beruházni és irántuk elköteleződni” (Antonovsky 1990, 160).
Az eredmények azt mutatták, hogy a bármilyen okból bekövetkező halálozás esélye ma gas masteryérzés esetében 15%kal, magas koherenciaérzés esetében pedig 10%kal csök kent.
A kardiovaszkuláris halálozásra a masteryérzés volt specifikus (rate ratio = 0,83), a koherencia nem csökkentette ennek esélyét (rate ratio = 0,97); utóbbival a daganatos megbetegedés alacsonyabb előfordulása járt együtt.
A koherenciaérzés szerepét elsősorban a szívbetegek életminőségével való össze
függésben vizsgálják (pl. julkunen–Ahlström 2006; skrabski és mtsai. 2006; Dantas et al. 2002), ennek a témának az ismertetése azonban meghaladja tanulmányunk kereteit.
