55 éves nőbeteg otthonából érkezik hozzátartozók kíséreté-ben. 8 éve kezelik diabetes mellitus miatt. Osztályos felvéte-lére eddig alkalmazott gyógyszeres terápiájának revíziója, va-lamint az időközben a bal láb öregujján kialakult fekély miatt került sor. Neuropathiás tünetekről számol be.
Időnként egyensúlyzavar is fellép a betegnél, vérnyomása rendszerint magas (150-160/90-100 Hgmm körüli mért érté-kek), gyakori fejfájást említ.
Vizelete rendben, alkalmankénti vizeletcsepegésről számol be.
• naponta kontrollálja a beteg testsúlyát
• az utasításoknak megfelelően előírt, a tápláltsági állapot és a vércukorháztartás ellenőrzésére vonatkozó vérvételek kivitelezését hajtsa végre
2. A szervezet salakanyagainak kiürítése
Probléma:
a betegnek gyakran van szorulása, vizeletcsepegés is előfordul
• rostokban és folyadékban gazdag táplálkozás biztosítása
• orvosi utasításra székletlazítók adása vagy beöntés kivitelezése
• szükség szerint inkontinenciabetét biztosítása
• intim női torna megtanítása gyógytornász bevonásával
• a kontinenciaproblémák áthidalása (éjszakai inkontinenciabetét biztosítása)
• orvos utasítása szerint fájdalomcsillapító és/
vagy vérnyomáscsökkentő készítmények adása, dokumentálása
5. A test tisztántartása
Probléma:
testsúlyából adódóan a mozgása nehezített és ebből adódóan a higiénés szükségletének kielégítése is
• segítségnyújtás az ápoló részéről a testi higiénés szükséglet kielégítésében (naponta tusolás, 3 naponta hajmosás, hetente pedikűr és manikűr biztosítása)
• a beteg motivációjának folyamatos fenntartása 6. A környezetben rejlő veszélyek elkerülése
Probléma: az egyensúlyzavar és a neuropathia miatt balesetveszély áll fenn a betegnél később (2-4. napig) támogató és végül (osztályos bennlét alatt) felügyelő szerepe legyen a beteg mozgása során
• nővérhívó elhelyezése elérhető helyre
• a beteg megtanítása a segédeszközök biztonságos használatára
7. A többi emberrel való kapcsolattartás Probléma:
a beteg egyedül, családjától külön él, így a látogatásokra ritkán van lehetőség, ami annak a veszélyét rejti magában, hogy a betegnek nem lesz elegendő alkalma a problémák megbeszélésére a családtagjaival
a családi kapcsolatok minőségének folyamatos fenntartása, javítása

a beteg haszontalannak érzi magát izikai korlátainak fennállása miatt
a haszontalanság érzésének megszűnése néhány órán belül
