III./8.1.: Classification of disorders of the vertebral column and the spinal cord

This chapter summarizes the diseases of the vertebral column and the spinal cord according to the following classification.

Classification based on the origin of disorders:

1. Developmental malformations of the spinal cord 2. Spinal trauma

3. Vascular diseases of the spinal cord

4. Inflammatory disorders of the vertebral column and the spinal cord

5. Metabolic myelopathies

6. Degenerative disorders of the spine 7. Spinal tumors

Different anatomical parts of the spine may be affected by a disease

Some of these disorders may involve several anatomical structures of the spine (e.g. degenerative diseases may simultaneously affect the vertebrae, the intervertebral discs, the facet joints, and the ligaments).

Others affect only one structural unit (e.g. myelopathy is the disease of the spinal cord).

Disorders of the vertebral column may lead to the secondary damage of the spinal cord and nerve roots, whereas primary spinal cord diseases rarely affect the vertebrae or the intervertebral discs.

The bony parts of the vertebral column are well seen by X-ray, but the spinal cord and nerve roots are

Arnold-Chiari malformation is often associated with hydrocephalus (enlargement of the ventricles), syringomyelia (a cavity formed within the spinal cord), and meningocele (a neural

Half of these injuries are associated with sever neurological deficit below the level of the injury, predominantly with anterior spinal cord

Spinal arterio-venous malformation (AVM) – chronic ischemia Vascular malformations in the spinal canal (dural arterio-venous fistula, malformation) - due to the shunting effect –

Inflammatory disorders can affect the vertebral column (vertebrae, intervertebral discs, facet joints and ligaments), the meninges of the spinal space, the nerve roots and the

Common causes of slowly developing spinal cord compression include cervical medial discal herniation, ostheoarthrosis of the vertebrae, thickened posterior longitudinal ligament,

Extradural tumors are primarily vertebral metastases, leading to vertebral compression and paravertebral – epidural spread, which causes spinal cord or nerve root

The aim of this learning unit is to provide a classification of the disorders of the vertebral column and the spinal cord, and to discuss the clinical signs and symptoms,
