Introduction

It is a challenge to determine the overall prevalence of muscle diseases, but - based on the prevalence of individual muscle disease groups - it is estimated to be 60-80/100,000. This estimated number of patients is roughly 10% of the number of patients suffering from cerebrovascular disorders, 50% of the number of patients with Parkinson’s disease, and slightly lower than the number of patients suffering from multiple sclerosis. It is even more problematic to determine how many patients suffer from systemic autoimmune diseases where the muscles are also involved. The significance of muscle diseases becomes evident when it is considered that

approximately every 25th patient of all neurological patients suffers from muscle disease.

Our aim is to present this topic from a practical point of view. After reading this chapter and completing the self-assessment tests, the reader should be able to use this knowledge in everyday medical practice. The classification of disorders is based on three different aspects: 1, functional anatomy; 2, physiology and pathophysiology;

3, biochemistry. It is essential to have a sound understanding of disease classification before going into the details of individual diseases. Proper understanding of disease classification and the underlying functional anatomical, physiological, pathophysiological, and biochemical aspects is reached by reading this part of the chapter several times. It is also important to have a good knowledge of basic neurophysiological diagnosis in order to understand the differential diagnosis of muscle diseases. In the third part of the chapter, therapy of muscle diseases is discussed, which is based on information presented in previous parts of this chapter.

Before reading this chapter, it is important to review certain information learned from previous studies. Without this, learning and understanding new information will be difficult or even impossible.

What should be reviewed? Please, recap your knowledge of the striated muscle and the neuromuscular junction (neuromuscular

synapse) from your textbook of neuroanatomy. Furthermore, go over again the principles and the application of neuromuscular diagnostic methods (ENG, EMG). These will also be discussed in detail in the diagnostic part of this chapter. Review also the histopathologic basis of muscle diseases in your textbook of pathology. After reviewing the information mentioned above, you are ready to start studying this chapter.

The time needed for each chapter is around 60-90 minutes, and please try to complete the self-assessment tests in 30 minutes.
